Opportunistic infections such as tuberculosis (TB) and serious bacterial, fungal, and parasitic infections are the most common causes of death among adults with advanced HIV disease.[1] The coinfection of both microorganisms has been documented in patients living with the Human Immunodeficiency Virus (HIV), in the AIDS phase[2]. There are very few data about coinfection in patients who are not immunocompromised. We present a clinical case of a young adult patient who presented with frank respiratory insufficiency, had previously been diagnosed with mycobacterial infection, upon reassessment of the patient it was decided to study hepatosplenomegaly due to pancytopenia, with liver biopsy showing Disseminated Histoplasmosis. treatment according to guidelines with Amphotericin B, and anti-phimic management with quadruple therapy. With evident clinical improvement, the patient has almost completely recovered his lung and kidney function and his homeostasis in general.
