Pediatric neurovascular disease has a unique pathophysiology: a) aorto-cardiac embolism; b) vascular disease; and c) hypercoagulability. At the onset, symptoms could be unspecific, or could present with local neurologic signs. An acute onset of focal neurologic signs implies a vascular origin until proven otherwise. However, it is impossible to determine if the symptoms are from an hemmorrhagic or ischemic etiology using only the clinical presentation. Perhaps, if the event has ocurred in seconds, it could be because of an embolic phenomenon; in minutes, thrombosis; and in hours, hemorrhagic. The differential diagnosis includes the "mimics", other diseases presenting with acute, local, neurologic signs. We present our clinical guideline for the diagnosis and treatment of ischemic cerebrovascular disease in children.
